Pre-Training Questionnaire

By submitting the private training client questionnaire, I recognize that the Ottawa Canine School does not guarantee behaviour change as behaviours are not static. I understand that behaviour modification requires patience, commitment, and consistency on my part, as well as following the training guidelines and recommendations set by my private training. In choosing to pursue private training to address my dog’s behavioural issues(s), I understand that I am incurring risk to myself and others and that it is my responsibility to use appropriate safety measures at all times, including muzzling and leashing my dog in the presence of other people and animals. I recognize that I am responsible for the actions of my dog at all times and I hereby agree to indemnify and hold harmless the Ottawa Canine School of any and all claims of injury, expense, costs, or damages caused by the actions of my dog. I further recognize that legal information is not the same as legal advice and that it is my responsibility to seek professional legal advice regarding my situation if needed. I understand that I will follow my private trainer’s instructions during the visit to ensure his or her safety. I agree to the Ottawa Canine School’s Private Training terms and conditions.
